Chapter 27: During Surgery, Did We Administer Anesthetic?
Konoha Hospital.
Emiya Shigen was here.
Now, he fully embodied the role of a doctor.
This was what Shigen needed to do in Konoha—to work as a doctor saving lives in the hospital. After all, he was a bona fide medical ninja.
Although most in Konoha didn't recognize his face, Shigen made no effort to hide his identity. Still, some of the staff at Konoha Hospital found him oddly familiar.
Many of the Senju clan's medical ninjas had, as children, snuck into the chamber holding Shigen's crystal coffin, much like Hashirama had done.
"Make sure he follows a strict diet."
The green chakra fading from his fingertips, Shigen didn't even look at the patient he had just healed. Instead, he gave instructions to an assistant standing nearby.
"Tell his family to clean up his room in the ward. He can be discharged today."
Chakra…
It truly was a miraculous force.
With chakra, wounds could heal rapidly.
When combined with expert surgical skills and vast medical experience, chakra's healing abilities could minimize even the most complex surgical risks.
"Lord Emiya is incredible!"
"We didn't even dare attempt surgery on that patient!"
"That's normal," Shigen replied.
"During surgery, you must maintain the patient's vitals. Excessive blood loss from large surgical wounds can lead to death, especially if the operation takes too long. And even with post-surgical medical ninjutsu, healing deep incisions can be challenging…"
While speaking, Shigen wrote on a medical chart, then handed it to the assistant.
"I've just done so many surgeries that I've grown used to it. The conservative treatment plan you employed wasn't wrong either—it at least kept him alive."
"What's this?" the assistant asked curiously.
"It's a list of potential medicines for treatment," Shigen explained casually. "I've seen many patients and learned a lot from their cases.
"For instance, the last patient I treated used a local herb to alleviate pain. It worked well and didn't cause additional complications for others who tried it."
"However…" Shigen added, "once you find the herb, don't administer it right away. Bring it to me first so I can study it. Every patient's condition is different."
Such knowledge stemmed from Shigen's ancient experiences.
Over a millennium had passed since his time. Humanity's biology may have evolved, so Shigen couldn't be entirely certain of any herb's effects without verification.
Outside the Operating Room.
Just as Shigen opened the door, he found Hashirama Senju kneeling in front of it.
"Lord Shigen!"
Hashirama bowed deeply, his forehead touching the floor. The First Hokage, weeping openly, begged Shigen to save his comrade, Sarutobi Sasuke.
"I implore you to save Sasuke! Please! I'll do anything you ask if it means he can survive!"
"…I see."
Shigen's brow twitched as he looked at the scene before him.
This man…
Why did he look like he was here to make trouble?
And the way he said "save Sasuke" felt oddly unsettling!
Shigen had known Hashirama was referring to Sarutobi Sasuke, yet it still sounded strange every time he heard it.
Behind Shigen, the assistant covered their face in embarrassment.
Everyone in Konoha knew the First Hokage. Despite his immense strength, Hashirama was notorious for being overly emotional, often crying or bowing when pleading for help.
"Sasuke went after Madara…"
Hashirama explained that Sarutobi Sasuke had been severely injured by Madara's Fire Release techniques.
"He's barely alive," Shigen muttered after examining Sasuke's wounds.
"!!!"
The group standing nearby was visibly alarmed.
"We'll need to operate immediately," Shigen continued.
"It's a shame he can't hold out for a few more days… I could've attended to him later."
Shigen sighed as Sasuke was brought into the operating room.
"Luckily, the next patient needing surgery has a few years left."
"..."
Everyone present broke into a cold sweat.
Wait…
Was this guy really a legitimate medical ninja?
Would Sasuke have had to wait in line if he could've lasted longer?!

The doors to the operating room closed.
Hashirama's guards quickly pulled him aside and forced him to sit down.
As they anxiously awaited the results of the surgery, a young boy with a Konoha forehead protector arrived. His face showed worry and tension.
"Monkey!" Hashirama called out, beckoning the boy over.
The boy was none other than Sarutobi Hiruzen, Sasuke's teenage son. At around 14 or 15 years old, Hiruzen was understandably distressed over his father's critical condition.
Because the Sarutobi clan's summoning beasts were monkeys, and due to his surname "Sarutobi," Hiruzen's mischievous and energetic behavior earned him the affectionate nickname "Monkey" from elders in the village.
"Hokage-sama, my father…"
Hiruzen hesitated, his voice trembling as he approached Hashirama.
"Don't be afraid, Monkey."
Hashirama reassured him, gently patting his head.
"The one treating your father is the greatest medical ninja in history…"
"Hiruzen!"
Several other teenagers came running down the hall, their expressions filled with concern. They bowed briefly to Hashirama before turning to Hiruzen.
"Hiruzen, we heard your father…"
Hiruzen nodded solemnly, but as he saw his friends' anxious faces, his own demeanor grew calm.
"Thank you. I appreciate your concern," he said steadily.
"Hokage-sama just told me that my father will recover soon."
"Of course!" Hashirama said with a bright smile, raising a hand in a vow.
"As Hokage, I promise that he'll be healed!"
"Hokage-sama!"
His guards panicked, rushing to cover his mouth.
This wasn't something he could casually guarantee!
Sasuke had been brought in barely alive—burned severely by Madara's fire techniques. That he had even survived long enough to reach the hospital was miraculous.
Then again…
Considering who his opponent was, it was already impressive that Sasuke had been brought back alive at all.
"Wait, where's Danzo?"
One of the girls suddenly noticed their group was missing a member.
"Danzo's probably still training at the grounds," a bespectacled boy replied.
"He's been working hard lately. We didn't have time to tell him about Hiruzen's father."
"Don't tell Danzo today," Hiruzen interjected, speaking with an air of maturity beyond his years.
"Once my father's condition stabilizes, I'll tell him. There's no need for Danzo to worry right now."
The other kids nodded in agreement.
Hiruzen's composure and leadership made him a respected figure among his peers.
Meanwhile, inside the operating room…
Shigen remained calm and focused on the surgery.
To him, this burn treatment was straightforward compared to the more complex internal surgeries he had recently performed.
To his assistant, however, the scene was nerve-wracking.
Shigen's left hand wielded a chakra scalpel, carefully removing charred skin and damaged muscle. Blood immediately flowed from the wounds.
Simultaneously, his right hand emitted green healing chakra, stopping the bleeding and rapidly regenerating tissue.
"Don't worry," Shigen reassured his assistant as he worked.
"The patient's body is strong."
"Compared to surgeries involving internal organs, this is relatively simple…"
"Fortunately…"
"The burns were caused by a Fire Release technique rather than prolonged exposure to flames."
"The patient was smart—there's no evidence of inhalation burns."
"If he had suffered inhalation burns on top of these injuries, the risk would've been much greater.
"I once treated a fire victim whose throat and lungs were horrifyingly burned…"
"And… what happened?" the assistant asked, clearly nervous.
"Hmm?" Shigen glanced at them briefly, then responded matter-of-factly:
"At the time, I was stronger than I am now. I simply cut open his throat and lungs to repair them."
"..."
The assistant felt dizzy hearing this.
"Lord Shigen… truly remarkable…"
A faint voice emerged from the operating table.
Sarutobi Sasuke, still conscious despite the pain, managed to squeeze out a word of praise.
"You're awake?"
Shigen paused for a moment, then suddenly asked:
"Wait… did I remember to give him anesthetic before starting the surgery?"
